OffSiteCare Financial Models
OffSiteCare offers three tiers of services. These range from providing an ICU Medical Director and associated services via telemedicine, to full-time onsite intensivist services supported by specialists through telemedicine. Each program includes a $20,000 startup fee to offset expenses for physician recruitment, credentialing, pre-implementation site visits, training, technology expenditures (control station and laptop remote control stations), procedures and protocol development, and legal expenses. Once established, the typical monthly charge for ICU/Medical Directorship is $5,000 per month.
1. Tier One Services. This service is initiated with a $20,000 startup fee, and costs $5000 per month. OSC provides robotic telemetry consultations in the ICU and ER when called, and does not provide patient follow-up. This includes 24/7 consultations with an intensivist and an acute stroke specialist
2. Tier Two Services. Includes all Tier One services as outlined above, and for an additional fee of $100 per/ICU bed per/day we will see and follow all patients. This fee is based on the number of ICU beds that are currently in use at the facility, and can be extended to cover additional beds on an as-needed basis.
3. Tier Three Services. Includes all Tier One and Tier Two services, and adds the services of an on-site intensivist from 7 AM to 7 PM, Monday through Friday. The intensivist will also provide coverage during off-hours via remote telepresence using the InTouch robot and working with on-site hospitalists. The cost for this service is $1000 per day.
This program has been successfully implemented at one of our Northern California sites, which is a closed unit that meets the highest specifications for an ICU. All patients are seen by intensivists, and plans are underway to create an on-site stroke center.
The remote telepresence solution is typically based on an InTouch Technologies RP-7 robot, which is leased directly from InTouch Technologies. We have found InTouch to provide an outstanding level of support which is included in the lease arrangement.
In all OSC billing models, we bill for what we do. Doctors receive 80% of what they bill, 10% goes to OSC, and 10% goes to EA TeleHealth for providing billing services.
